Risk factors and indications for stem cell transplantation in MDS

Sergio A. Giralto, MD, professor of medicine at Weill Cornell Medical College, attending physician in the Division of Adult Bone Marrow Transplantation at Memorial Sloan Kettering Cancer Center, vice chief of the Division of Hematological Malignancies, and the Melvin Berlin Family Chair in Multiple Myeloma at Memorial Sloan Kettering Cancer Center, will discuss current indications for considering stem cell transplantation in patients with myelodysplastic syndromes (MDS).

Additionally, Zirart is looking more closely at how factors such as disease risk classification and patient characteristics influence this. Zirart has written about this topic in: 2024 Society of Hematology and Oncology (SOHO) Annual Meeting.

0:09 | Currently, the indications for transplantation in patients with MDS are those with intermediate-to-high-risk disease or those who have not responded to first-line treatment and remain transfusion-dependent, and in whom transplantation is thought to significantly improve the quality and quantity of life.

0:28 | [It is] This is essential to stratify the risk of patients, and we have found that some patients who are considered low risk because of molecular abnormalities are actually high risk patients because they have a higher propensity to progress to acute leukemia, and we have also found that some patients who are assessed as intermediate risk may actually have a lower propensity to progress.

[composite health risk assessment model] The score can look at patients over age 65 and predict their risk of death after transplant. These types of scoring and risk classification criteria are very important for both the patient and the treating physician to determine whether the risk-benefit ratio of transplant is positive or whether the patient would be better off with a more conservative approach.
