Study participants were identified from the Diagnostic Procedure Combination (DPC) Inpatient Database, a Japanese administrative claims database. This database includes more than 1,000 of his hospitals, including all 82 university hospitals, and covers more than half of all inpatient admissions in the country.16. In 2020, there were 338,256 pneumonia-related cases in the DPC database, either as the primary diagnosis during hospitalization, as the reason for hospitalization, or as the disease requiring the highest medical costs. Inclusion criteria for this study were patients aged 18 years or older, length of hospital stay longer than 24 hours, and primary diagnosis of hospitalization being either his COVID-19 or pneumonia (Figure 1). S1). COVID-19 pneumonia and non-COVID-19 pneumonia were recognized by ICD-10 codes. A number of patients including second or subsequent hospitalization, death within 24 hours of admission, BMI <15 or >50, incomplete information on BMI, ADL, type of admission (urgent or non-urgent), and patients with aspiration. The primary outcome of this study was the occurrence of in-hospital death from any cause. The secondary outcome was decline in physical function, defined as at least a 20% reduction in Barthel Index score at discharge compared with admission. Propensity scores were estimated using a logistic regression model. Risk of mortality or functional decline was estimated using logistic regression analysis between patients after matching, adjusting for age, sex, BMI, Barthel Index score at admission, and Charlson Comorbidity Index score .
